The Mechanism
For most of the 20th century, medicine was certain that stomach ulcers came from stress, spicy food, and excess acid; the notion that bacteria could survive in the acid bath of the stomach was dismissed as absurd. In Perth, Australia, the pathologist Robin Warren kept seeing curved bacteria in gastric biopsies, and a young physician, Barry Marshall, became convinced these microbes — later named *Helicobacter pylori* — were the true cause of gastritis and many ulcers. Unable to reproduce the disease in lab animals and unable to persuade skeptical colleagues, Marshall, then 32, at Fremantle Hospital, took an extraordinary step: after a baseline endoscopy confirmed his stomach was healthy, he drank a broth teeming with bacteria cultured from a patient. Within days he was nauseated and vomiting; a repeat biopsy on day eight showed his stomach lining was now inflamed and colonized by *H. pylori*. He then cured himself with antibiotics. The self-experiment helped overturn a century of doctrine: most peptic ulcers were an infection that could be cured with a short course of antibiotics rather than merely managed for life. Marshall and Warren shared the 2005 Nobel Prize in Physiology or Medicine.
Why It Matters
For decades, stomach ulcers seemed to fit an acid-and-stress story, so the idea that bacteria could live in the stomach felt impossible. The surprise is that the stomach was not the real mystery - the real mystery was whether a microbe could survive there and cause disease. Robin Warren's microscope findings and Barry Marshall's risky self-experiment helped show that Helicobacter pylori could inflame the stomach lining and trigger many ulcers. That shifted treatment from simply reducing acid to using antibiotics to clear an infection.
Wait — That's Not Quite Right
A common mistake is to think ulcers are mostly caused by spicy food, worry, or having too much stomach acid. Those things can irritate symptoms, but they are not the main cause of most peptic ulcers. In many cases, the key problem is Helicobacter pylori infection, which damages the stomach lining and can be treated with antibiotics.

Map Your Stomach Story
Take a sheet of paper and draw a simple outline of a stomach. On one side, write the old explanation people once believed - stress, spicy food, and too much acid. On the other side, write the newer explanation - infection with Helicobacter pylori. Then draw arrows showing how a germ can inflame the stomach lining and lead to an ulcer.
Next, look at a nutrition label or menu and list three foods people often blame for stomach pain. Then, with an adult, talk about the difference between something that can irritate symptoms and something that actually causes disease. The goal is not to diagnose anything at home, but to practice separating a common belief from the evidence that changed medicine.
